125cc atv chain coming off

So I’ve got a 125cc atv and I’ve tightened the chain where it has little slack and it shouldn’t come off.I thought it was the front sprocket so I changed it but it still fell off.So I checked the carrier bearings and they have little play on them but it’s like literally nothing.One thing I did notice was the Hubs where loose could this cause this.Also every time the chain is going to come off I hear like this clunking noise.Any help is appreciated Thanks

Chain adjustor

MEASURE from the LEFT front rim... to the left rear rim... and do the same on the right side.... adjust the spread so you are WITHIN 1/2 inch spread... and 1/2 inch deflection on th e chain... all will be good... tighten the 4 bolts good and tight
